The process for requesting information from the Division of Environmental Health is:

• Complete an application for Public Access to Records.

  Freedom of Information Form (.doc format) Freedom of Information Form (.pdf  format)

• Submit application by mail, fax or in person.
• Once approved, you may review records in the Office of Environmental Health, Yeager Health Center, Building D, Pomona, NY.
