Mission Statement
To provide services to the physically disabled adults residing in Rockland County that will stave off premature institutionalization regardless of financial status.

We provide structured and comprehensive programs that provide a number of services including healthcare monitoring, therapy, case management, and therapeutic recreation in a safe and cheerful environment.

Why an Adult Day Healthcare Center?
There are many men and women throughout Rockland who do not need or wish to live in an institution. As a result, we pioneered this medically oriented day care program. 
However, by living at home, their medical and rehabilitation needs are not being met.
Our Adult Day Healthcare program, runs Monday through Friday from 8 AM to 3 PM.
We utilize a skilled nursing and rehabilitation day-care approach, enabling participants to live-at-home. Essential medical, rehabilitation and other services restore and/or maintain a person at their maximum level of function for optimum quality of life.

What services do you provide?
Those accepted into the program will have the opportunity to participate in an individual program which include the following services:

bullet Medical, functional, nursing and psychosocial evaluations to determine the participants needs and plan a course of treatment.
bullet We work closely with the participants’ personal physician.
bullet Rehabilitation programs including occupational, physical and speech therapy.
bullet Nursing and dietary counseling.
bullet The family and the participant have access to social services, including case work, group work services and referrals to other resources if necessary.
bullet The center also provides a recreational program geared to meet the needs of the participants.

How do you identify appropriate services to provide?

bullet At the beginning, participants are evaluated to identify medical, functional and psychosocial levels and needs.
bullet Based on the results of these evaluations, an individual care plan is drawn up. Family members and the participant are part of the care plan process.
bullet As individual needs change, the care plan is evaluated to assess whether it should be altered.
bullet We work closely with personal physicians. As needed, we arrange for dental services, podiatry, laboratory and nutrition counseling.
bullet In addition, should the needs of the participant change, we will provide the essential help through expanded assistance.

How do you address quality of life issues?

bullet Our rehabilitation programs include, but are not limited to - occupational, physical and speech therapy.
bullet Access to social services, including casework, group work services and referrals to other sources if necessary.
bullet The center also provides a nutritious meal and recreational programs geared to the needs of the participants.

How can someone participate if they are unable to drive?
We provide free transportation from home to the center and back.

How much does this cost?
Fees depend upon the number of days of participation and are charged on a sliding scale. In addition, Medicaid covers the cost. This will be discussed and explained at your initial meeting.
